Domino Flap Using Bilobed Flap for Large Scapular Flap Donor Site: A Case Report

INTRODUCTION Scapular flaps favored for large soft tissue defects but high risk of shoulder contracture. This case report presents novel application of bilobed flap technique to cover donor site after harvesting 23 cm scapular flap. CASE PRESENTATION A-25-year-old male with right hand open degloving injury measuring 25 x 22 cm. Scapular flap planned to cover soft tissue defect and use the bilobed flap to close the donor site. After six month, left shoulder showed full range of motion and the scapular flap is viable with good right-hand function. DISCUSSION This case introduces the largest bilobed flap used to date. The report emphasizes two main advantages, it prevents shoulder contracture and allows surgeons to exceed the traditionally recommended flap size with minimal donor site morbidity CONCLUSION Large bilobed flap effectively closing donor site defect after scapular flap harvest, yielding satisfactory results.
